Job summary

The Advanced Practice Pharmacist (APP) at Akido Companies is responsible for comprehensive medication management across primary care and specialty practices, collaborating with physicians, nurses, and care teams to optimize therapy and improve patient outcomes.

The role requires a PharmD, active license, CA APP designation or eligibility, and at least two years of ambulatory care experience, with strong communication and education skills.

Qualifications

• PharmD required and an active license in the applicable state.
• APP designation or eligibility per CA regulations.
• Minimum 2 years clinical pharmacy experience in ambulatory, primary, managed care, or related settings.
• Experience using EHRs and clinical documentation systems.
• Excellent communication and patient education skills.

Responsibilities

• Provide comprehensive medication management services for patients with chronic and complex conditions.
• Evaluate and optimize medication regimens using evidence-based guidelines.
• Initiate, modify, monitor, and discontinue medications per regulations and agreements.
• Conduct patient assessments and identify drug therapy problems.
• Monitor outcomes, safety, and adherence; support population health initiatives.
• Collaborate with physicians and care teams to develop personalized treatment plans.
• Counsel patients on medications, diseases, and self-management.
• Document interventions and recommendations in the EHR.
• Participate in quality improvement efforts.
