Akido Labs, Inc. seeks an Advanced Practice Pharmacist (APP) to provide comprehensive medication management across primary and specialty care. You will work with physicians, nurses, and care teams to optimize therapy, improve outcomes, and support population health initiatives.

The APP leads evaluations, adheres to collaborative practice agreements, and documents interventions in the EHR. Requires PharmD, California license, and relevant ambulatory care experience.

Qualificações

• Pharmacist with strong clinical and communication skills.
• Experience applying evidence-based guidelines to patient care.
• Comfort with electronic health records and care coordination.
• Ability to work with physicians and care teams across settings.

Responsabilidades

• Provide comprehensive medication management for chronic and complex conditions.
• Evaluate and optimize regimens per guidelines and protocols.
• Initiate, modify, monitor, and discontinue medications per agreements.
• Conduct patient assessments and reviews to identify therapy problems.
• Monitor effectiveness, safety, adherence, and outcomes.
• Collaborate with interdisciplinary teams to plan individualized care.
