PRIMARY DUTIES:
  • Provide advanced clinical services including performing athorough medication reconciliation and assessment to identify,prevent, and resolve medication-related problems.
  • Provide advanced clinical services for individuals with acuteand chronic pulmonary and sleep-related breathing conditions,including, but not limited to, asthma, chronic obstructivepulmonary disease (COPD), bronchiectasis, interstitial lung disease(ILD), pulmonary hypertension, pulmonary arterial hypertension(PAH), obstructive and central sleep apnea, and other complexcardiopulmonary disorders.
  • Perform comprehensive medication reconciliation and assessmentto identify, prevent, and resolve medication-related problems.Recommendations are documented during the clinic encounter andcommunicated to the clinic provider or, when appropriate, to thepatient's outside healthcare providers. Follow-up appointments arescheduled as clinically necessary.
  • Manage discharge medication needs and provide medicationeducation for patients with COPD who are admitted to BWMC andsubsequently discharged for post-hospital follow-up in thepulmonary clinic.
  • Provide transitional care pharmacy services to support patientstransitioning from the hospital to the outpatient clinic, includingparticipation in rounds, clinic visits, and interdisciplinarytreatment team meetings.
  • Evaluate the clinical and financial impact of specialtymedications and high-cost pulmonary therapies; identifyopportunities to optimize site of care, medication acquisitionstrategies, formulary selection, and benefit utilization.
  • Participate in quality improvement initiatives and clinicalpathway development to enhance service quality, productivity, andinstitutional best practices.
  • Provide institutional service through committee participationand other activities that support the hospital and pharmacyprogram.
  • Precept PharmD students and pharmacy residents and contributeapproximately 10% effort to the School of Pharmacy PharmDcurriculum including on-site assignments.
  • Provide quality improvement services through engagement inadvanced pharmaceutical scholarly and research activities,including conducting or participating in research and disseminatingfindings through peer-reviewed publications and presentations atlocal, regional, and national meetings; provide written ordelivered reviews on current topics as appropriate.
  • Collaborate with interdisciplinary teams and contributeclinical expertise to support optimal patient care, medication use,and pharmacy practice.
  • Perform other duties as assigned within the scope of thisrole.
Qualifications :
MINIMUM QUALIFICATIONS:

Education: Bachelor's degree in Pharmacy. Doctoral degree inpharmacy (PharmD) preferred.

Experience: Two (2) years of experience in a pharmacy,OR One (1) year post-baccalaureate or post-doctoral/post-graduatepharmacy residency (PGY1) plus one (1) year of experience, OR Two (2) years post-baccalaureate or post-doctoral/post-graduatepharmacy residency (PGY2).

Certification/Licensure: Current Maryland licensure in goodstanding to practice as a pharmacist or a federal districtlicensure where the work will be performed upon hire.

Certification in a relevant specialty by the Board of PharmacySpecialties or Commission for Certification in Geriatric PharmacyBoard such as Certified Psychiatric Pharmacist (BCPP) orBoard-Certified Geriatric Pharmacist (BCGP) eligibilitypreferred.

Other: Candidates selected for employment may be subject tomedical inquiries and/or medical examinations to determine abilityto perform the job. Employees in this job class may be subject topre-employment and random drug testing.

KNOWLEDGE, SKILLS, AND ABILITIES:

Comprehensive knowledge of pharmaceuticals, their indications,interactions and potential side effects. Knowledge andunderstanding of relevant physiology, pathophysiology and diseasestates. HIRING RANGE:

$125,000 - $143,000 per year / Commensuratewith education and experience
