Job summary

The Aleda E. Lutz VA Medical Center in Saginaw, MI seeks an Advanced Practice Nurse (APN) specializing in Urology.

This on-site role emphasizes managing a patient panel, coordinating care across the team, ordering labs, and educating patients and families to preserve high-quality outcomes. Principals include leadership within the interdisciplinary team, adherence to ICARE values, and provision of outpatient and inpatient urological services.

Responsibilities

• Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
• Develops and implements an individualized plan to achieve optimal population health and disease management
• Demonstrates leadership and works collaboratively with the interdisciplinary team
• Orders and interprets labs, tests, and diagnostic studies
• Prescribes pharmacologic and non-pharmacologic interventions
• Initiates referrals and consults to the specialty services
• Provides education to patients and their families
• Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
• Provides urological services to Veterans - both outpatient and inpatient at the Aleda E. Lutz VA Medical Center
• Other duties as assigned
