The Veterans Health Administration - Hampton Virginia VAMC, Geriatrics and Extended Care Service, North Battlefield VA Clinic, is seeking a registered nurse to provide comprehensive nursing care within a multidisciplinary team.

The role emphasizes patient assessment, care planning, coordination of specialty referrals, and patient education to support optimal outcomes for patients with complex needs. This on-site position offers federal benefits and a structured work schedule.

Aufgaben

• Responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care (ANA Code of Ethics for Nurses)
• Provides a full range of nursing care to patients with complex care needs utilizing the nursing process based on the ANA Standards of Nursing pratice
• Utilizes clinical assessments to develop plans of care - which reflect therapeutic - educational - preventative - and rehabilitative aspects of care
• Performs a comprehensive patient assessment including health history - physical examination - evaluations of deviations from the norm and age specific disease screening - immunizations - and health counseling
• Formulates differential diagnosis and initiates appropriate intervention based on physical findings
• Refer Veterans to specialty clinics when indicated
• Advises - teaches - and counsels individuals - families - and groups regarding identified health care needs and provides information for prevention of illness and injury
• Collaborates with other disciplines to ensure continuum of care
• Completes a comprehensive and timely discharge summary within the parameters of Medical Center Policy
• Discharges patients from care in a timely and effective manner
