NPACE in Jackson, MS, is seeking a Nurse Practitioner to provide a full range of services within Community Care Service, focusing on quality, timeliness, and VA mission advancement. The role includes clinical practice, consultation, and management across patient populations.

You will collaborate with patients, families, and healthcare teams to implement care plans, participate in performance improvement, and utilize evidence-based guidelines to optimize outcomes.

Responsibilities

• Independently practices as a NP providing a full range of services utilizing treatment guidelines in the delivery of high-quality care
• Collaborates with patients, families, and healthcare practitioners to develop, implement, and evaluate care
• Utilizes customer service principles, supports veteran self-determination, and serves as a resource for patients and staff in addressing ethical issues
• Contributes to performance improvement initiatives by identifying, analyzing, developing, and implementing processes that improve care at the panel, program, or population level
• Incorporates research, evidence-based practice, established professional, community, or regulatory standards, and clinical practice guidelines to validate or change clinical practice
• Supports dissemination of evidence-based practice, i.e., research, quality improvement monitors, best practices, and performance improvement results
• Identifies and evaluates clinical/process opportunities and trends for improvement and takes appropriate action for resolution
• Collaborates with stakeholders to improve patient care, staff development, and performance measures
• Demonstrates positive, practical communication skills and professional behaviors that promote cooperation and teamwork with patients and other customers
• Develops and implements an individualized plan to achieve optimal population health and disease management
• Demonstrates leadership and works collaboratively with the interdisciplinary team
• Orders and interprets labs, tests, and diagnostic studies
• Prescribes pharmacologic and non-pharmacologic interventions
• Initiates referrals and consults to specialty services
• Provides education to patients and their families
• Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes

The Advanced Practice Nurse (APN) Nurse Practitioner (NP) practices in Community Care Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission as well as provides advanced clinical practice - consultation - and management.
