Job summary

U.S. Department of Veterans Affairs is seeking an Advanced Nurse Practitioner (APN) in Chesapeake, Virginia. The APN will provide advanced clinical practice, consultation, and management within the Veterans Health Administration. Advanced Nurse Practice (APN) Position

The Advanced Nurse Practice (APN) position is located within the Department of Veteran Affairs (VA) – Veterans Health Administration (VHA) – under the Mental Health and Behavioral Sciences Service Line (MHBS). The NP will provide advanced clinical practice, consultation, and management. This announcement is open continuously until closed.

Duties
  • Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
  • Develops and implements an individualized plan to achieve optimal population health and disease management.
  • Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members.
  • Orders and interprets diagnostic studies.
  • Prescribes pharmacologic and non-pharmacologic interventions.
  • Initiates referrals/consults to specialty services.
  • Provides education to patients and their families.
  • Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
