The facility is dedicated to serving diverse communities with a team‑based approach to comprehensive care, proudly serving a largely migrant and Latino community with certified bilingual medical assistants.

Opportunity

Advanced Practice Clinician – Nurse Practitioner or Physician Assistant

Job Details
  • Full‑time position (1.0 FTE, 40 hours per week) providing comprehensive family medicine care
  • Serve underserved patient populations in a community health setting
  • Manage your own patient panel with opportunities to develop and cultivate long‑term patient relationships
  • Work with certified bilingual medical assistants for translation and patient care support
  • Utilize EPIC EMR system for patient documentation and care coordination
  • Clinic operational hours: Monday‑Friday 7:30 AM – 7:00 PM
  • Participate in a team‑based approach to patient care within a patient‑centered medical home model
  • Opportunity for 4‑week Medical Spanish immersion program in Xela, Guatemala
  • Must complete a Master’s or Doctoral degree as a Nurse Practitioner or a Master’s degree as a Physician Assistant
  • New graduates encouraged to apply with clinical rotation experience and community health background
  • HPSA Score for Primary Care: 18
