**Responsibilities**

Lead Advanced Heart Failure and VAD program APP

The Advanced Heart Failure and VAD APP will work as part of a multidisciplinary team providing comprehensive care to patients with advanced heart failure, including those receiving guideline-directed medical therapy, and mechanical circulatory support (e.g., LVAD).This role includes mostly inpatient and some outpatient responsibilities and close collaboration with cardiologists, cardiac surgeons, VAD coordinators, and heart failure teams.

+ Provide advanced clinical management of VAD patients in both inpatient and outpatient settings.

+ Coordinate preoperative evaluations, patient selection, and readiness for VAD implantation.

+ Serve as primary liaison between patients, families, physicians, and the VAD team.

+ Manage and coordinate follow-up care, including VAD monitoring, device troubleshooting, and medication management.

+ Educate patients, caregivers, and healthcare professionals on VAD care, safety protocols, and lifestyle modifications.

+ Participate in the development and implementation of VAD program policies, procedures, and clinical guidelines.

+ Ensure compliance with regulatory and reporting requirements (e.g., INTERMACS, CMS, The Joint Commission)and maintenance of DNV accreditation.

+ Collaborate with multidisciplinary teams including cardiology, cardiothoracic surgery, social work, and palliative care.

+ Participate in on-call rotation to provide 24/7 support for VAD patients.

+ Contribute to quality improvement projects and research related to mechanical circulatory support

Qualifications:

+ Education: Master’s degree in nursing (NP) or completion of accredited PA program.

+ Licensure: Current licensure as an NP or PA in IL; BLS and ACLS certification required.

+ Experience: At least 1–2 years of cardiology experience preferred. Prior experience in heart failure, LVAD, or transplant strongly preferred.

+ Skills: Excellent clinical judgment, communication, and teamwork skills. Ability to manage complex cardiac patients independently with physician collaboration.

**Requirements**

Licensure / Certification / Registration:

+ BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date required. Credentials are verified by local staff credentialing office

+ Nurse Practitioner credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required. Credentials are verified by local staff credentialing office.

+ Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred.

Education:
