The Advanced Practice Clinician in the Jump Start Assessment (JSA) program is a physician-led and advanced practice clinician-driven program that is intended to care for and support Alignment Healthcare patients with complex and chronic care needs. It works within an interdisciplinary team environment that includes physicians, nurse practitioners, physician assistants, case managers, and other healthcare team members to ensure proper delivery of clinical and home-based patient care. The role involves assessing, developing, and coordinating options and services promoting quality care to achieve optimal health outcomes while ensuring cost-effective care in compliance with company policies and regulations. Members are typically aged 65 and over, with home and virtual visits lasting 45 to 60 minutes depending on the visit type.

Responsibilities
  1. Conduct in-home assessments (with some outpatient clinical work) on health plan members.
  2. Include obtaining comprehensive history, physical exam, medication review, and appropriate cognitive/depression/safety screenings during assessments.
  3. Engage in end-of-life care discussions as needed.
  4. Collect laboratory specimens in the home setting when appropriate.
  5. Identify diagnoses for active medical and care management, focusing on chronic disease management.
  6. Communicate assessment findings to inform the PCP of potential care gaps.
  7. Provide education to patients, families, and caretakers, emphasizing close monitoring and follow-up.
  8. Refer to community resources as appropriate.
  9. Maintain compliance with HIPAA regulations and ensure the security of PHI.
Experience

Job Requirements:

  • Required: None
  • Preferred: At least 1 year of clinical or home care experience; EMR experience preferred; experience with older adults preferred.
Education
  • Required: Master’s degree from an accredited NP or PA program
Specialized Skills
  • Excellent communication skills, professionalism, and effectiveness in interactions.
  • Strong problem-solving, organizational, and time management skills; ability to work in a fast-paced environment.
Licensure
  • Active California Nurse Practitioner or Physician Assistant license required.
  • RN license and Furnishing number (NP only) required upon hire.
  • Board certification in Nurse Practitioner or Physician Assistant certification required.
  • NPI, DEA, BLS, California driver’s license, and current automobile insurance required.
Physical Requirements
  • Regularly required to talk, hear, stand, walk, sit, handle objects, and reach.
  • Frequently lift/move up to 20 pounds; vision and focus abilities needed.

Pay Range: $113,332.00 - $169,999.00
