Job summary

ChristianaCare's Thrive Practice is seeking an experienced NP or PA to deliver comprehensive, patient-centered primary care for medically and psychosocially complex adult patients in a collaborative team. The role emphasizes individualized care plans, care coordination, and ongoing professional development.

The ideal candidate has 3+ years in primary care, Delaware licensure, and APRN or PA-C certification, with strong communication and advocacy skills to support population health initiatives

Qualifications

• Graduate of an accredited Nurse Practitioner (Adult, Adult-Gerontology, or Family Medicine) or Physician Assistant program.
• Minimum 3 years of experience in primary care, internal medicine, family medicine, or complex care management.
• Current Delaware licensure and board certification as APRN or PA-C.

Responsibilities

• Deliver comprehensive primary care to adult patients with complex medical and behavioral health needs.
• Develop and manage individualized care plans focused on patient goals and outcomes.
• Coordinate care across healthcare settings and community resources.
• Educate and counsel patients and families using evidence-based teaching strategies and teach-back methods.
• Collaborate with physicians, care managers, and other healthcare professionals to optimize patient care.
• Participate in quality improvement, population health, and value-based care initiatives.
• Maintain accurate and timely documentation within the electronic health record (EHR).
• Support onboarding and precepting of APCs, students, and other learners as needed.
• Promote an exceptional patient experience through compassionate, relationship-based care.
