Responsibilities

• Performs complete history and physicals and documents in the medical record.
• Collaborates with interdisciplinary team to diagnose and manage acute and complex chronic conditions.
• Formulates and documents a plan of care addressing complex health care needs and implements evidence‑based interventions.
• Documents progress and communicates with patients and families to ensure continuity of care.
