Job summary

The Central Iowa VA Healthcare Facility in Des Moines, IA is recruiting for a Full-Time APN - Pulmonary to join the Pulmonology Service. The Advanced Practice Nurse will provide a full range of clinical services with emphasis on health promotion and disease prevention within a Whole Health System of care.

The role emphasizes independent patient management, consultation support for pulmonology outpatients, and collaboration with the interdisciplinary team to deliver high-quality Veterans Affairs

Responsibilities

• Independently manage patients with acute and chronic health conditions with emphasis on health promotion and disease prevention using evidence-based practice
• Provide consultation and follow-up for pulmonology patients in the outpatient setting at the VA Medical Center in Des Moines
• Develop and implement individualized plans to achieve optimal population health and disease management
• Demonstrate leadership and collaborate with the interdisciplinary team
• Order and interpret labs, tests, and diagnostic studies
• Prescribe pharmacologic and non-pharmacologic interventions and initiate referrals to specialty services
• Educate patients and families and adjust care plans as needed to achieve outcomes
• Act as surrogates for pulmonology colleagues when they are on leave and work within privileges

Duties

• The Advanced Practice Nurse practices in Pulmonology Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission
• The NP provides advanced clinical practice - consultation - and management

• Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
• Most providers will need to act as surrogates for the patients of their pulmonology colleagues when one or more of their pulmonology colleagues are on leave
