Job summary

Mercy Hospital St. Louis is seeking a Primary Care Patient Care Advanced MA to work with Mercy Primary Care Providers in delivering safe, timely, patient-centered care. You will collaborate with patients and the care team to drive quality outcomes in an outpatient setting.

The role requires a high school diploma, completion of MA competencies, and MA Clinic Academy training, with strong skills in medication reconciliation and care-gap closure.

Qualifications

• High school diploma or equivalent.
• New hire must attend MA Clinic Academy training.
• Must have completed all MA Competencies.

Responsibilities

• Complete Advanced Standard Rooming with overall achievement >= 90% for last 6 months including AWV questionnaire and documentation.
• Thorough Medication Reconciliation.
• Closing all open care gaps as indicated by Encounter Guide.
• Blood Pressure Recheck for patients with elevated readings.
• Completion of Patient Experience Conversation.
• Proficiency with EHR systems and clinical documentation.
• Ability to multitask and adapt in a fast-paced clinical environment.
