Job summary

The Jeffrey M. Carlton Heart and Vascular Institute at Hoag is seeking an experienced advanced practice provider to help build a comprehensive advanced heart failure program from the ground up.

The APP will work with cardiologists, surgeons, intensivists, pharmacists, nurses, and care managers to develop an integrated model of care across hospital and outpatient settings. Qualifications

• MSN or DNP required.
• Proctoring by Medical Staff Office completed.
• Advanced clinical knowledge in care and management of patients.

Responsibilities

• Inpatient assessment and management, new heart failure consultations, multidisciplinary rounds and post‑hospital follow‑up.
• Outpatient visits, support of physician clinics, medication optimization and patient education.
• Coordinate with referring clinicians and ensure a transition of care with a clear treatment plan.
