Qualifications

• 2+ years of experience as a practicing NP or PA-C.
• Comprehensive health assessment capabilities.
• Experience in Internal Medicine, Primary Care, Urgent Care, Emergency, and Family Medicine preferred.

Responsibilities

• Partner with CMOs for patient assessments and diagnosis.
• Collaborate to manage and optimize treatment plans.
• Educate patients on health promotion and disease prevention.
• Coordinate patient care with interdisciplinary healthcare teams.
• Maintain accurate medical records per legal standards.
• Participate in home visits as needed.
