Job summary

Cleveland Clinic Health System in Mayfield Heights at Hillcrest Hospital is seeking an Advanced Practice Provider (NP/PA) for Advanced Care Planning. The ACP Team collaborates with physicians and care teams across Fairview, Lutheran, Hillcrest, and Euclid locations to guide goals-of-care discussions.

Hours are 8:00am–5:00pm, Monday to Friday, with no holidays or weekends. The role emphasizes patient values, education on directives, and coordinating goals with the care plan.

Qualifications

• Nationally certified APRN (NP/PA) registration in the state.
• MSN required upon hire or within 3 years for external hires and internal hires moving from a non-APRN licensed role to an APRN.
• Experience in advance care planning or goals-of-care discussions preferred.

Responsibilities

• Advance Care Planning consultations with patients and families.
• Facilitate discussions on healthcare goals, values, preferences, code status, and decision-making.
• Educate patients and caregivers about advance directives, healthcare power of attorney, and end-of-life planning options.
• Document patient wishes in the electronic health record.
• Collaborate with primary and specialty providers to align treatment with patient goals.
