Job summary

Qualifications

• Master of Social Work degree from CSWE-accredited program.
• Licensed Social Worker credentials (LSWAIC/ASWA/BSW/LICSW) may be required or preferred.
• Strong communication and advocacy skills.

Responsibilities

• Provide social work assessments and interventions for hospitalized and emergency department patients.
• Conduct crisis intervention and patient/family support.
• Coordinate post-acute placement and connect with community resources.
• Arrange patient/family meetings with health care team.
• Educate staff and families on care planning and resources.
• Identify risks and refer for services as needed.
