Job summary

Sweetser in Sanford, ME is hiring a Targeted Case Manager - Adult to support individuals across the care continuum. The role emphasizes outreach, case management, and helping people access health care and community services.

The position offers an hourly rate of $22.00-$24.00, a $5,000 sign-on bonus, and a chance to join a team recognized for service excellence. Will work with the Integrated Behavioral Health team within a CCBHC framework.

Qualifications

• Bachelor's degree in Human Services or related field preferred, but not required.
• Maintain a valid state driver's license.
• MHRTc Certified or eligible.
• Experience working in or with a primary care clinic encouraged.

Responsibilities

• Conduct assessments of participants with guardian or supports for health and social needs.
• Provide information on health care options and alternatives to hospitalization.
• Coordinate and monitor Individual Treatment Plans within the CC BHC team.
• Determine level of care using screening tools.
• Assist with health insurance applications and housing resources as needed.
• Provide in-person support at the CCBHC walk-in center.
• Develop wellness plans for self-management and coordinated services.
• Document services in EHR and ensure compliance with guidelines.
