Overview

Adult Protective Services Intake Case Manager 2 (APS Central Intake Specialist) - 00017116

Under general supervision, gathers referral information from reporters via telephone interviews, written reports, or face-to-face contacts; assesses referral information and makes a determination as to whether the situation meets Adult Protective Services (APS) criteria; provides intervention services if the situation does not meet APS criteria.

Responsibilities
  • Gathers referral information from reporters via telephone interviews, written reports, or face-to-face contacts.
  • Assesses APS referral information and determines whether the situation meets APS criteria.
  • Provides Information and Referral Services on situations that do not meet APS criteria.
  • Documents pertinent referral information in Harmony in a thorough, accurate, and clear manner.
  • For reports meeting APS criteria, documents as an Investigation.
  • For reports that do not meet APS criteria, documents as an Intervention.
  • Clears the name of the person referred in Harmony to determine if there is an active case or history.
  • Notes findings in Harmony.
  • Completes an acknowledgement letter to the reporter unless receipt of the letter is waived.
  • Saves the letter in the Central Intake Folder on the computer.

Qualifications
  • OR One (1) year of experience at APS Intake CM 1 (SSP180) or position equivalent.
Preferred Skills/Qualifications
  • Two (2) years’ experience as an APS Investigator.
  • Experience working with client populations facing issues such as family violence, crisis intervention, substance abuse, or services to the elderly or disabled.
  • Bachelor’s degree in a Human Services program or related field.
  • Possession of a valid driver’s license and consistent access to a vehicle for required work-related activities.
