Adult Nurse Practitioner - LGBTQ+ Primary Care

Adult Nurse Practitioner position in Norwalk, CT, offering full-time outpatient primary and preventive care within a welcoming, inclusive practice serving the LGBTQ+ community.

Provide care for approximately 15 patients/day, including physical exams, chronic disease management, and preventive screenings.

Offer gender-affirming services such as hormone therapy, collaborating with physicians and behavioral health providers.

Deliver culturally competent, patient-centered care across the lifespan.

Conduct assessments, follow-ups, and collaborate with a multidisciplinary team.

Engage in community outreach and health education; Medical Spanish is a plus.

Work Monday-Thursday (9am-5pm) and Friday (9am-4pm), with no call.
