Adult Inpatient Mental Health Care Manager

Join to apply for the Adult Inpatient Mental Health Care Manager role at Fairview Health Services.

This position partners with physicians and the interdisciplinary team to guide patients and families through the treatment process, including assessment, treatment planning, discharge planning, and community re-entry. Responsibilities include managing expectations, responding to hospitalization crises, supporting discharge and re-entry needs, and monitoring patient length of stay and avoidable days.

Job Responsibilities
  1. Coordinate treatment planning and manage each case.
  2. Develop individualized treatment plans in collaboration with the team, considering age, social and cultural factors, diagnosis, and functioning levels.
  3. Implement, reassess, and adjust plans as needed.
  4. Assess patient needs through interdisciplinary tools, updating assessments weekly.
  5. Partner with the team to develop and communicate discharge plans, ensuring they are culturally and clinically appropriate.
  6. Conduct group, family, and individual therapy sessions, documenting thoroughly and timely.
  7. Collaborate with Utilization Management to monitor care quality and reimbursement.
  8. Promote evidence-based practices through participation in related activities and projects.
  9. Respond to behavioral control situations and participate in CPR as required.
Organizational Expectations

  • Partner with caregivers in decision-making.
  • Maintain a safe and respectful environment.
  • Provide patient education tailored to learning needs.
  • Complete all organizational and legal requirements.
Qualifications

Education: Master’s degree in psychology, counseling, social work, marriage and family therapy, or related field.

Experience: At least two years in therapy, case management, or assessment, providing face-to-face services to adults or children/adolescents.

Licenses/Certifications: BLS certification and one of the following: LISW, LICSW, LGSW, LP, LPC, LPCC, LAMFT, LMFT.

Preferred Qualifications

Master’s in Social Work and LICSW certification.
