Job summary

You will develop individualized transition plans, arrange resources, and provide ongoing guidance while collaborating with participants and partners to foster growth and independence.

Qualifications

• Bachelor’s Degree required.
• Strong commitment to helping young people and adult learners succeed.
• Awareness of and sensitivity to diversity and values.

Responsibilities

• Conduct strengths-based assessments to explore assets and factors influencing self-sufficiency.
• Co-create individualized transition plans reflecting progress and goals.
• Identify, develop, and arrange resources to support plan implementation.
• Provide ongoing follow-up and guidance, building on strengths to overcome barriers.
• Advocate within human service, health, court, and criminal justice systems.
• Coordinate retention efforts and assist with recruitment for an inclusive environment.
• Teach career, parenting, job readiness/skills, and leadership development.
• Connect participants with employment opportunities and college coursework.
