Waypoint Maine, Inc. is seeking a skilled Case Manager to support adults with intellectual disabilities/autism and their families in the Western Maine area. The role involves identifying needs, developing care plans, coordinating services, and advocating for ongoing support in collaboration with a multi-disciplinary team.

Hybrid remote work is available after onboarding. Our team emphasizes a collaborative approach with small caseloads and opportunities for supervision to achieve and maintain

Qualifications

• BA/BS in human services or related field is required or preferred.
• BA/BS in unrelated field plus minimum 1 year experience in adult IDD services.
• Knowledge of the Maine system of care is preferred.

Responsibilities

• Identify needs and develop a plan of care with the individual.
• Initiate referrals and monitor services for progress.
• Coordinate and advocate for services identified in the plan and support licensure goals.
