Job summary

Community Healthcare Network is seeking a qualified social worker to perform adolescent social assessments and provide services to patients and families. The role includes developing and implementing life skills plans, coordinating referrals, and delivering short-term, goal-oriented counseling within a supportive, confidential environment.

The position emphasizes HIV counseling, prenatal/family planning involvement, and collaboration across CHN services to enhance patient outcomes.

Qualifications

• MSW required or equivalent.
• LMSW licensure required or eligible for LMSW within state guidelines.

Responsibilities

• Assess adolescents' social and emotional needs and collaborate on a plan.
• Provide concrete services to clients and families or help secure them.
• Document services in the medical record.
• Carry out short-term counseling with a minimum caseload.
• Identify and connect clients with external resources and monitor effectiveness.
• Offer HIV universal counseling with pre/post-test support as needed.
• Engage in prenatal, family planning and primary care psychosocial assessments.
• Deliver health education through individual and group sessions.
• Participate in staff development opportunities.
• Coordinate confidential services for CHN patients and staff.
• Serve on committees or task forces; co-facilitate group work for special populations.
