Qualifications

• Must be licensed in Arizona as LAC/LMSW/LAMFT/LPC/LCSW/LMFT.
• Current fingerprint clearance card issued by the Arizona Department of Public Safety.
• CPR and First Aid certification kept current.
• No suspension or debarment from federal health programs.

Responsibilities

• Develop and implement personalized treatment plans.
• Provide group and individual therapy for teens and adults using evidence-based practices.
• Lead PHP group therapy program five days a week.
• Mentor advanced graduate interns.
• Collaborate with a multidisciplinary care team.
• Attend in-person at the designated Gilbert office.
