Job summary

You will collaborate with psychiatric providers and therapists to ensure a cohesive plan of care and actively engage families in treatment decisions.

Requirements include a Master’s Degree in Social Work and Minnesota licensure, plus healthcare and EHR experience.

Qualifications

• MSW required with Minnesota licensure.
• Healthcare experience preferred.
• Experience using electronic health records preferred.
• Proficient in Microsoft Office (Word, Outlook).

Responsibilities

• Assessment and Planning: conduct thorough assessments, maintain the care plan, and lead groups.
• Patient and Family Engagement: maintain open communication with patients and families to involve them in decisions.
• Ethical Advocacy: deliver patient-centered, culturally appropriate, and age-appropriate care aligning with standards.
