Job summary

The Institute for Family Health in New York, NY seeks a School-Based Health Education and Access Coordinator to support adolescent health at SBHCs. You will deliver education sessions, collaborate with SBHC staff, and screen for public health insurance programs to improve access to care.

Qualifications

• Bachelor’s degree in social work, psychology, public health, health education or related field.
• 1–2 years of health education and/or insurance access/enrollment experience.
• Familiarity counseling adolescents on reproductive health.
• Familiarity with health insurance issues.
• Excellent organizational and customer service skills.
• Able to manage multiple projects simultaneously.
• Detail oriented and thorough; able to work independently or in a small team.

Responsibilities

• Conduct individualized health education during SBHC appointments.
• Make referrals to medical provider and social work for all new patients.
• Maintain chart documentation; clinical care supervised by collaborating clinician.
• Screen children and families for Medicaid, CHP, FHP, FBPB, PCAP and WIC eligibility.
• Completes the Access NY application for eligible families and tracks enrollment and billing metrics.
• Collaborate with SBHC staff to develop and implement health curricula.
• Conduct outreach and follow-up with students and families.
• Commit to at least two years in the position.
