Job summary

Amedisys is seeking an Admissions Registered Nurse to coordinate hospice admissions and develop initial care plans for patients at home. This role requires a valid RN license in the applicable state, plus CPR certification and a minimum of one year of RN experience. Expect travel mileage and a salary of $85,000–$87,000 with benefits.

Responsibilities

• Coordinate admissions by assessing patients' physical, psychosocial and spiritual needs.
• Establish initial plan of care with attending physician, medical director, and interdisciplinary team.
• Provide skilled interventions to manage symptoms and ensure patient comfort under physician direction.
• Act as liaison between disciplines and facilities during admission.
• Maintain and submit accurate care progress records and documentation.
• Communicate findings and changes in patient condition to the clinical manager/physician and hospice team; obtain orders as required.
• Perform on-call responsibilities and on-call services to patients/families.
• Perform other duties as assigned.

Responsibilities

• Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs.
• Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team.
• Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician.
• Functions as a liaison between disciplines and facilities during the admission process.
• Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate.
• Communicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.
