• Minimum of 12 months experience in acute care or home health.
• Experience in Home Care including OASIS preferred.

Responsibilities

• Complete admissions and visits for Home Care.
• Deliver nursing care as directed by RN Case Manager.
• Document patient services and create plans of care.

A Few Must-haves For This Position

• Licensed to practice as an RN in the state of Minnesota. License is active and in good standing.
• Able to travel within geographic area serviced by the office. Valid driver's license, reliable transportation, and proof of auto insurance required.
• Minimum of twelve (12) months experience working in an acute care or home health environment.
• Evidence of current CPR certification.
• Experience in Home Care to include OASIS; minimum one (1) year experience preferred.
  
  

Some Functions Of What You Would Do

• Completes admissions and/or visits for the Home Care team.
• Responsible for delivering nursing care to patients as directed by the RN Case Manager and the physician’s plan of care.
• Ability to assess and determine appropriate wound care, including obtaining MD orders.
• Provides accurate and timely documentation of patient services to reflect and create the plan of care.
• Provide and determine treatments for the patients.
