Emmanuel Hospice is seeking an Admissions Registered Nurse (RN) in Grand Rapids, MI and surrounding areas. This full-time role involves the first clinical contact for patients entering hospice, providing compassionate support and admissions in homes, hospitals, or facilities.

The position offers a sign-on bonus, mileage reimbursement, generous time off, day-one benefits, and a supportive team. Travel locally is required as part of patient admissions.

Responsibilities

• Schedule admissions shifts (Th, Fri, Sat 9a–9p).

• Educate, perform assessments, and coordinate with physicians on eligibility.
• Explain hospice services, goals, and benefits clearly.
• Complete documentation and consent forms promptly.
• Coordinate with the interdisciplinary team for smooth transitions.

We are currently seeking a caring and experienced Admissions Registered Nurse (RN) to join our team and as the welcoming presence for patients and families beginning their hospice journey.

Responsibilities include:

• Schedule: Th, Fri and Sat 9a-9p
• First clinical contact for patients and families entering their hospice journey while providing emotional support and reassurance
• Conducting initial hospice information sessions and admissions in patients’ homes, hospitals, or facilities
• Performing education, clinical assessments and collaborating with physicians on hospice eligibility
• Clearly explaining hospice services, goals, and benefits to patients and their families
• Completing all required documentation and consent forms in a timely manner
• Coordinating with the interdisciplinary team to ensure a smooth transition into hospice services

Qualifications:

• Current RN license in the state of Michigan
• Minimum of 2 years clinical experience, preferably in hospice, palliative care, or acute care
• Exceptional interpersonal and communication skills
• Ability to explain medical and hospice-related information with compassion and clarity
• Strong organizational and documentation skills
• Valid driver’s license, reliable transportation, and willingness to travel locally
