Job summary

American Senior Communities is seeking an Admission Nurse (RN) at East Lake Nursing & Rehabilitation Center in Elkhart, IN. This management-oriented role runs 10am–6pm with on-call rotation and emphasizes compassionate, patient-centered care within a community-focused setting.

The RN will coordinate resident care across shifts, monitor new admissions, and support discharge planning while upholding CARE values and HIPAA requirements. Licensed RN in Indiana and geriatric experience preferred.

Qualifications

• Graduated from an accredited school of nursing and hold a current Indiana RN license.

Responsibilities

• Coordinate resident care between shifts to ensure continuity of care and communicate with the care team.
• Monitor admissions to ensure appropriate care plan interventions.
• Oversee evening admissions and transitions to the health care community.
• Coordinate and manage evening/weekend discharges.
• Provide education and training to direct care givers on changes in condition.
• Monitor residents with falls or new/worsening behaviors to maintain continuity of care.
