A leading healthcare organization in Warwick is seeking a field clinician to manage patient care from admission to community transition. The role involves coordinating care, monitoring treatment, and ensuring compliance with regulations. A Bachelor's Degree is preferred for this position.
Under the supervision of the Team/Program Manager, the field clinician is responsible for managing a caseload of patients from admission to transition into the community. This involves interdisciplinary support and referrals as appropriate. The role includes coordinating interdisciplinary care, monitoring and adjusting treatment to achieve positive therapeutic outcomes, and identifying opportunities for process improvement. All responsibilities will be performed in accordance with organizational policies, procedures, and standards, complying with Federal, State, and accreditation regulations.
Equivalent education required for Rhode Island licensure; a Bachelor's Degree (minimum) in a relevant discipline is strongly preferred.
