Job summary

Bristol Hospice is seeking an Admissions RN to establish relationships with customers and referral sources in East Georgia. You will promote hospice access, coordinate admissions, and collaborate with ED and DPCS to plan care.

The role requires an active GA RN license, BSN or equivalent experience, and willingness to travel about 20% with flexible hours to ensure timely patient admissions and high-quality care.

Qualifications

  • Must hold an active RN license in Georgia.
  • Must have at least 1 year of nursing experience in the last 3 years or a BSN from an NLN-accredited program.
  • Willingness to travel up to 20% and flexible hours.
  • Proficiency with common nursing documentation and information systems.

Responsibilities

  • Engage with patients, families, and referral sources to promote hospice access.
  • Coordinate admissions and consults, ensuring timely care transitions.
  • Collaborate with ED, DPCS, and community education to support care planning.
  • Complete initial patient/family assessments and develop care plans.
  • Serve as liaison between facilities, physicians, hospitals, SNFs, and hospice teams.
