Job summary

Heart to Heart Hospice is seeking an Admissions RN in Texas to perform initial patient assessments and coordinate hospice services with the care team. The role requires a current RN license, a valid driver’s license with reliable transportation, and 1–3 years of clinical, hospice, or home health experience.

You will review referrals, obtain consent, develop an initial care plan with the team, communicate with the Intake Coordinator and managers, ensure timely documentation, and uphold Medicare

Qualifications

  • Graduate of an accredited nursing program (Diploma/Associate/Bachelor).
  • Current State RN license.
  • Valid driver's license and reliable transportation.
  • 1–3 years of RN experience in clinical care, hospice or home health preferred.

Responsibilities

  • Demonstrate autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
  • Review clinical referral information and respond to requests and inquiries as appropriate.
  • Collaborate with Patient Care Manager, attending physician and hospice physician regarding eligibility and appropriateness of client for hospice services.
  • Conduct initial meeting with the patient and family over hospice service and consent for care.
  • Provide the initial comprehensive assessment of physical, psychological, social and spiritual needs of patient and family and initiate interventions.
  • Develop an initial care plan with immediate goals and interventions, based on nursing assessment and collaboration with the attending HMD and core team.
  • Maintain regular communication with the Intake Coordinator, Patient Care Managers, RN Case Manager and core team to review patients and manage needs.
  • Timely submission of all documentation.
  • Performance of other duties as required.
  • Ensure compliance with local, state and federal laws, Medicare regulations, and personnel policies and procedures.

What You Must Have:

  • One to three years of experience as an RN in a clinical care setting, hospice or home healthpreferred.

  • Timely submission of all documentation
  • Performance of other duties as required
  • Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
