Job summary

Regional Hospice is seeking an experienced Admissions RN to coordinate patient care in the community and at Danbury Hospital. This full-time role involves admitting patients, assessing needs, and developing care plans in collaboration with families and physicians.

The successful candidate will have an active CT RN license, at least two years of nursing experience (Hospice/Home care preferred), and may hold a CHPN certification. A comprehensive benefits package is offered.

Qualifications

• Graduation from an accredited school of nursing; BSN preferred.
• Current CT RN license is required.
• Minimum of two years' nursing experience, preferably in medical/surgical settings.
• Hospice/home health or end-of-life care experience preferred.
• Active client contact within the past three years required.

Responsibilities

• Admit patients to hospice services.
• Evaluate prospective hospice patients as needed.
• Make visits to patients as needed.
• Provide skilled and supportive nursing care to patients and families.
• Perform assessments of patient and family, identifying needs.
• Develop and communicate the care plan, and complete required paperwork.
• Coordinate with attending physician and staff; identify additional home health services.
