Job summary

BAYADA Home Health Care seeks an Admissions Registered Nurse (RN) for full-time home health visits across Monmouth, Middlesex, Union, and Somerset Counties, NJ. You will perform comprehensive SOC admissions, nursing assessments, and develop plans of care with the interdisciplinary team.

Responsibilities

• Complete comprehensive OASIS SOC admissions visits for home health clients in the assigned territory.
• Conduct thorough initial nursing assessments, including medication reconciliation and safety evaluations.
• Develop individualized plans of care in collaboration with physicians, clients, caregivers, and the team.
• Coordinate nursing, therapy, social work and other services as needed.
• Educate clients and caregivers on diagnoses, medications, and home health plan of care.

BAYADA Home Health Care is seeking an experienced Admissions Registered Nurse (RN) for a full-time opportunity with our UCV office, providing home health admissions and OASIS Start of Care (SOC) visits to adult and geriatric clients throughout Monmouth, Middlesex, Union, and Somerset Counties, NJ.

As an Admissions RN, you'll play an important role in welcoming new clients to BAYADA and setting them up for a successful home health experience. You'll complete comprehensive initial assessments, establish individualized plans of care, and collaborate with our interdisciplinary team to ensure a smooth transition into home health services.

How You’ll Make an Impact:

• Complete comprehensive OASIS Start of Care (SOC) admissions visits for home health clients throughout the assigned territory.
• Perform thorough initial nursing assessments, including medication reconciliation, safety assessments, and evaluation of each client's clinical and functional needs.
• Develop individualized plans of care in collaboration with physicians, clients, caregivers, and members of the interdisciplinary team.
• Identify client needs and coordinate appropriate nursing, therapy, social work, and other home health services.
• Review and execute physician orders and ensure the plan of care accurately reflects each client's needs and goals.
• Educate clients and caregivers regarding diagnoses, medications, treatments, safety, and the home health plan of care.
• Communicate clinical findings and changes in condition to physicians and appropriate members of the BAYADA care team.
• Ensure timely and accurate completion of OASIS assessments and all required admission documentation.
• Accurately document assessments, interventions, education, and care coordination using BAYADA's electronic medical record (EMR) system.
• Help ensure a smooth transition from referral to admission and ongoing home health services.

Qualifications:

• Current and unrestricted New Jersey Registered Nurse (RN) license.
• Minimum of one year of recent, verifiable clinical nursing experience.
• Prior home health experience required.
• OASIS experience required.
• Strong clinical assessment and care coordination skills.
• Excellent communication, interpersonal, organizational, and time-management skills.
• Ability to work independently and effectively manage a field-based schedule.
• Experience with electronic medical records (EMR) preferred.
• Reliable transportation and ability to travel throughout Monmouth, Middlesex, Union, and Somerset Counties.

Additional Information:

Base Pay, depending upon qualifications, status, and subject to negotiation:

$65.00 - $75.00 per point
