Job summary

BAYADA Home Health Care is seeking an experienced Admissions Registered Nurse (RN) for a full-time role covering Monmouth, Middlesex, Union, and Somerset Counties, NJ.

You will perform comprehensive initial assessments, establish individualized care plans, and collaborate with the interdisciplinary team to ensure a smooth transition into home health services. This is a field-based role with autonomy and strong client relationships.

Qualifications

• Current and unrestricted New Jersey RN license.
• Minimum of one year of home health nursing experience.
• OASIS Start of Care (SOC) experience.
• Strong clinical assessment and care coordination skills.
• Ability to travel across Monmouth, Middlesex, Union, and Somerset Counties.

Responsibilities

• Complete comprehensive OASIS SOC admissions visits for home health clients in the assigned territory.
• Perform thorough initial nursing assessments, including medication reconciliation, safety assessments, and evaluation of client needs.
• Develop individualized plans of care with physicians, clients, caregivers, and the interdisciplinary team.
• Identify client needs and coordinate nursing, therapy, social work, and other home health services.
• Review physician orders and ensure the plan reflects client needs and goals.
• Educate clients and caregivers about diagnoses, medications, treatments, safety, and the home health plan of care.
• Communicate findings to physicians and BAYADA care team.
• Ensure timely completion of OASIS assessments and admission documentation.
• Document assessments, interventions, education, and coordination in EMR.
• Help ensure smooth transition from referral to admission and ongoing services.

How You’ll Make an Impact:

• Complete comprehensive OASIS Start of Care (SOC) admissions visits for home health clients throughout the assigned territory.

• Develop individualized plans of care in collaboration with physicians, clients, caregivers, and members of the interdisciplinary team.
• Identify client needs and coordinate appropriate nursing, therapy, social work, and other home health services.
• Review and execute physician orders and ensure the plan of care accurately reflects each client's needs and goals.
• Educate clients and caregivers regarding diagnoses, medications, treatments, safety, and the home health plan of care.
• Communicate clinical findings and changes in condition to physicians and appropriate members of the BAYADA care team.
• Ensure timely and accurate completion of OASIS assessments and all required admission documentation.
• Accurately document assessments, interventions, education, and care coordination using BAYADA's electronic medical record (EMR) system.
• Help ensure a smooth transition from referral to admission and ongoing home health services.

Qualifications:

• Current and unrestricted New Jersey Registered Nurse (RN) license.
• Minimum of one year of recent, verifiable clinical nursing experience.
• Prior home health experience required.
• OASIS experience required.
• Strong clinical assessment and care coordination skills.
• Excellent communication, interpersonal, organizational, and time-management skills.
• Ability to work independently and effectively manage a field-based schedule.
• Experience with electronic medical records (EMR) preferred.
• Reliable transportation and ability to travel throughout Monmouth, Middlesex, Union, and Somerset Counties.

Additional Information:

Base Pay, depending upon qualifications, status, and subject to negotiation:

$65.00 - $75.00 per point
