Job description

BAYADA Home Health Care is currently seeking experienced Physical Therapists (PT) for home health visits with adult clients in northern Ocean County. An immediate opportunity is available for an Admissions / Start-of-Care (SOC) Physical Therapist. Prior home health and OASIS experience are strongly preferred.

As a home care Physical Therapist, you will be a vital part of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities for a PT - Physical Therapist include:

1. Make home visits to clients in designated areas.
2. Perform diagnostic tests and measurements, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
3. Develop and implement individualized care plans, including manual therapy, gait training, balance, and other interventions.
4. Assess and revise care plans regularly, collaborating with nursing staff as needed.
5. Educate clients and families on rehabilitative activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using electronic tablets.
7. Implement wound prevention and repair interventions using evidence-based dressings and topical agents.

Qualifications for a PT - Physical Therapist include:

1. Current New Jersey Physical Therapist license.
2. Graduation from an APTA-approved program or equivalent, with proof via transcript or diploma.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Proficiency with computers; EMR experience preferred.
