Responsabilités

• Coordinate and complete nursing assessments for prospective and newly admitted residents.
• Review hospital records, diagnoses, physician orders, medications, and clinical documentation for admission appropriateness.
• Collaborate with hospital case managers, physicians, facility leadership, and interdisciplinary teams to coordinate timely admissions.
• Ensure admission documentation, medication reconciliation, and required assessments are completed accurately and timely.
• Identify clinical needs, potential risks, and care considerations prior to and upon admission.
• Communicate with residents and families to answer questions and establish a positive first impression.
• Ensure equipment, medications, treatments, and care resources are in place for new residents.
• Support census growth by providing responsive communication to referral partners.
• Maintain compliance with facility policies and regulatory requirements.
