Qualifications

• New York RN or LPN license required.
• 1–2 years of clinical nursing experience.
• Experience in nursing home admissions.
• Knowledge of CMS guidelines, DOH regulations, and insurance policies.
• Familiarity with electronic health records (EHR) systems.
• Strong communication and interpersonal abilities.
• Ability to perform physical assessments and interpret clinical data.
• Proficiency with computer systems and software applications.
• Ability to work in a fast-paced environment.

Responsibilities

• Interpret PRI/RUG Classification System accurately.
• Manage referral caseload and respond in a timely manner.
• Assess classification and patient disposition during screening.
• Coordinate referral via EMR including verification of benefits.
• Coordinate bed board/census when needed.
• Assess potential patients at partnering hospitals.
• Address age-specific physical, psychosocial and emotional changes in residents/patients when planning programs and delivering care.
• Maintain updated knowledge of services available for new admissions.
• Discuss/interprete patient potential and status with appropriate disciplines.
• Establish and maintain professional rapport with health care team members.
• Demonstrate strong communication with patients, families and all disciplines.
• Communicate clearly to residents, patients and families, considering cognition changes.
