Job summary

Quantum Rehabilitation and Nursing is seeking an Admissions Nurse to join our team in New York. The RN must admit new patients and uphold nursing standards in daily operations. The role requires strong communication, state compliance, and collaboration with interdisciplinary team.

Responsibilities include completing Admission Assessment, entering data into EMR, obtaining orders, and coordinating follow-up. The position offers full-time schedule M-F 3-11 with weekend as needed and pays $55 per

Qualifications

• Must be a Registered Nurse to apply.
• Experience with SigmaCare or similar EMR is preferred.
• LTC/STR experience is a plus.
• MDS language knowledge is beneficial.
• Good assessment skills are required.

Responsibilities

• Admit all new patients to the facility and ensure nursing operational standards.
• Complete Admission Nursing Assessment paperwork and enter data into EMR.
• Obtain orders from MD and ensure accuracy of Admission Orders in EMR.
• Communicate and collaborate with the interdisciplinary team and physicians.
• Complete the Admission Nursing Note and Fall Risk/Pressure Ulcer risk assessments.
• Ensure all paperwork for admission is complete and signed off.
• Coordinate follow-up consults for scheduling.
• Report to the RN Supervisor on duty.

The Admission Nurse is responsible for admitting all new patients to the facility while ensuring the nursing operational standards of the facility are met.

• Must have strong communication skills.
• Comply with all state requirements and facility policies, procedures and standards of conduct.
• Have the ability to assess, plan, implement and evaluate nursing care according to the individualized needs.
• Communicates and collaborates with the interdisciplinary team and or physicians.
• Completes all paperwork for the Nursing Admission Assessment.
• Enters all data into Electronic Medical Record.
• Obtains orders from MD and ensures accuracy of Admission Orders in EMR.
• Completes the Admission Nursing Note.
• Completes the Fall Risk Assessment and interventions care plans as indicated (sensors, alarms, etc).
• Completes the Pressure Ulcer Risk Assessment / Braden Scale. If high risk ensures all preventative measures are implemented.
• Obtain consents: Pneumovax / Flu and places in the patients’ chart.
• Ensures Initial/Interim Care plans are put in place. Fall Risk, Skin Alt-Potential, Pain-specific resident conditions (pressure ulcer, post-op, infection, etc).
• Administers 1st dose of PPD within 24 hrs and records on Immunization Record and Med sheet.
• Ensures all paperwork for the admission is complete and signed off on.
• Ensures that all follow-up consults are created for the Unit Secretary to schedule in a timely fashion.
• Reports to the RN Supervisor on duty.
