Job summary

Social Row Transitional Care seeks an admissions coordinator to efficiently obtain required information and admit residents in line with established policies and sound business practices.

Responsibilities include assessing care level needs, admitting residents, collecting signatures, explaining billing procedures, and entering admission data into the MDS system. You will coordinate transfers and bed availability to ensure smooth transitions for residents.

General Description

The primary purpose of this position is to obtain required information and admit residents in an efficient manner, in accordance with established policies and procedures, and sound business practice.

Essential Functions:

• Assess prospective admissions for level of care needed and cost to provide care.
• Admit and prepare identification records for residents in accordance with established policies and procedures.
• Assist in interviewing residents/responsible parties and obtaining required information and signatures.
• Assist in the orientation program for residents/responsible parties in accordance with established policies, make sure all documents are signed properly, and all billing procedures are explained fully.
• File pre-admission and admission documentation appropriately.
• Enter admission data into computer for MDS department usage.
• Ensure the resident’s room is prepared prior to arrival and notify all departments to residents ETA.
• Transfer and discharge residents in accordance with established policies and procedures, making appropriate changes to documentation and notifying appropriate departments.
• Maintain log of all admission and discharges, maintaining an accurate record of available beds.
• Develop marketing strategies to ensure relationships with outside referral sources. Utilize assessment time to market facility to hospital discharge planners.
• Visit hospitalized resident, and keep in contact with responsible party during hospitalization.

• Must possess the ability to make independent decisions, when circumstances warrant such action.
• Must possess the ability to deal tactfully with personnel, residents, family members/responsible party, visitors, government agencies/personnel and the general public.
• Must be well groomed and professionally dressed.
• Must be knowledgeable in the use of computers, data entry and monitoring.
• Must have personal integrity, flexibility and the ability to work effectively with residents, personnel and support agencies.
