Heather Health Care Center seeks an Admissions Director to lead the admissions process, ensuring compliant screening of referrals and accurate documentation in the EMR system. The role emphasizes coordination with hospital partners, families, and the corporate AR team to secure timely payments and maintain census targets.

The candidate will oversee enrollment workflows, supervise staff as needed, and promote strong customer service while staying in line with federal, state, and facility policies.

Qualifications

• Bachelor’s Degree in related field or 2 years direct experience.
• Preferred knowledge of long-term care.
• Communicates appropriately, adaptable to changes in assignments and priorities, and can delegate tasks when appropriate.
• Must work independently and manage outside agency contacts with tact and discretion.
• Ability to multi-task, meet deadlines in a challenging environment, and think analytically.
• Demonstrates positive customer service and respectful attitude.
• Technology oriented with working knowledge of Microsoft Excel and Microsoft Outlook, EMR systems and ability to learn new software.
• Preferred experience with insurance portals.

Responsibilities

• Communicate effectively with hospital and community agencies regarding admission referrals.
• Review resident eligibility for Medicaid/Medicare/Insurance before acceptance.
• Coordinate required documents and deposits for admission to ensure compliance.
• Schedule admission with family/resident and obtain signatures on necessary forms.
• Collaborate with Interdisciplinary Team to assess resident needs and feasibility of admission.
• Maintain census accurately in the EMR and update as needed.
