Job summary

Timberline Post Acute is seeking an Admissions Coordinator to manage the admissions process and help meet census goals. The role involves coordinating with physicians, social workers, and discharge planners to admit patients efficiently while ensuring accurate documentation and insurance verification.

The ideal candidate will have at least a high school diploma, with preferred Bachelor’s degree in health care and LVN/RN licensure.

Qualifications

  • High school diploma or GED is required.
  • Bachelor's degree in health care or related field preferred.
  • Two years' admissions experience preferred.
  • LVN or RN license preferred.
  • Ability to read and interpret general business materials and communicate clearly.

  • Manage admissions process to achieve census goals and provide a positive experience for residents and families.
  • Plan, direct, and monitor daily admissions operations and paperwork.
  • Admit patients and verify admission criteria and insurance coverage.
  • Coordinate with physicians, social workers, discharge planners, and other departments.
  • Maintain confidentiality of resident information and adhere to HIPAA rules.
  • Attend daily stand-up meetings and maintain bed availability records.
  • Support outreach activities to referents and maintain warm, service-oriented atmosphere.

General PurposeThe primary purpose of your job is to support facility operations by increasing the facility census.Essential DutiesManage the admissions process efficiently to ensure a positive experience for residents, families and supporting staff.Planning, directing and monitoring the daily operations of the Admissions Department to ensure that budgeted resident census goals are met and that residents are admitted in a professional and efficient manner.Admits Patients to the health care facility.Screens patients by comparing patients’ condition to admission criteria.Admits patients by completing admission and financial responsibility forms.Must have the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures of the Sales and Marketing department.Working collaboratively with leaders of other facility departments to ensure timely communication of appropriate information and to enhance the admissions experience of residents and their families.Confirms that all insurance benefit coverage meets standards of admissions.Coordination and arranging physical, social, emotional and support services requirements, including transportation.Obtain applicant information by requesting completed applications and medical information; verifying and clarifying information; interviewing patients and family members; explaining admission criteria.Maintains comprehensive understanding of facility services that can be provided to prospective residents and educates referral sources as appropriate.Oversees the patients bed assignments and completion of preminary paperwork for admissions.Obtain Applicant information by requesting complete applications and medical information; verifying and clarifying information; interviewing patients and family members; explaining admission criteria.Completes additional facility-specific ongoing tasks and projects as assigned by immediate supervisorCreate and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the facilityCompletes resident intake process. Coordinates with Director of Nursing and/or Case Manager to assure appropriateness of facility admissions. Maintains and communicates accurate record of bed availabilityWorks to initiate, nurture and maintain contacts with physicians, social workers, and discharge planners as valued customers to maximize referrals and facilitate admissions;Must attend daily stand-up meetings.Be familiar with Standard Precautions, Exposure Control Plan, Fire Drill and Evacuation Procedures and know how to use the informationMaintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.Protects organization reputation by keeping information confidential.As directed, assists in planning and execution of outreach events such as on-site facility health fairs, educational programs, and other promotional events designed to attract hospital discharge planners, physicians, and other key persons to the facility.Update job knowledge by participating in educational opportunities; reasing professional publications and maintaining personal networks.Supervisory RequirementsThis position has supervisor responsibilities.QualificationEducation and/or ExperienceMust possess, as a minimum, a high school diploma or GED.A Bachelor's Degree in health care or related field Preferred.Two years' Admissions experience preferred.LVN or RN license preferredLanguage SkillsAbility to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.Ability to write reports, business correspondence, and procedure manuals.Ability to effectively present information and respond to questions from managers and employees.Mathematical SkillsAbility to apply concepts such as fractions, percentages, ratios and proportions to practical situations.Reasoning AbilityAbility to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.Certificates, Licenses, RegistrationsTo preform this job successfully, an individual must be proficient in the Microsoft Suite productsPhysical DemandsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
