Job summary

Masonicare Health Center in Wallingford, CT is seeking a part-time Admissions Counselor to handle weekend admissions. The role processes admissions, assesses financial eligibility, and coordinates with the care team to ensure smooth transition and accurate documentation.

Responsibilities include responding to inquiries, evaluating care needs, guiding families, and maintaining waitlists while supporting PASSR/ASCEND workflows and insurance verification.

Qualifications

• Bachelor’s degree preferred.
• 3–5 years admissions experience with Medicare/Medicaid knowledge.
• Insurance verification experience.

Responsibilities

• Responds to admission inquiries via hospital, referral source, telephone or walk-in and maintains public relations with customers.
• Assess financial status, evaluates care needs, and assigns level of care for wait lists.
• Provides supportive care and guidance for continuing care needs.
• Ensures PASSR/ ASCEND process is completed for new residents and that financial eligibility has been met.
• Coordinates pre-admission/admission conference with care center team to transfer information.
• Screens referrals and coordinates acceptance of admission with care center team.
• Maintains waitlist for long-term care applications and works with Social Worker for processing.
• Signs in all long-term care applicants and distributes paperwork.
• Performs other duties as assigned.

The Admissions Counselor is responsible for the processing and admitting of individuals with services at Masonicare Health Center in accordance with financial policies and procedures.

Essential Duties and Responsibilities:

1. Responds to admission inquires via hospital, referral source, telephone or walk in and provides pertinent information.. Maintains and fosters public relations with customers, internal and external.
2. Assess individual/ financial status, evaluates care needs evidenced by medical/ functional information presented, and assigns level of care so that the applicant is placed on the wait list for the appropriate service area.
3. Provides supportive care and guidance to applicants and their families for continuing care needs for community or institutional services
4. Ensures PASSR/ ASCEND process is completed by appropriate staff for new residents for admission and ensures that the financial eligibility has been met.
5. Coordinates pre-admission/ admission conference with representative of the care center team, i.e., director, care coordinator, and social worker to ensure transfer of medical, social, and financial/ insurance information to the care team
6. Screens referrals of patients from hospital and community for admission to rehab/ sub acute units and coordinates acceptance of their admission with care center team, i.e., care coordinator, social worker, and clinical coordinator.
7. Maintains waitlist for applications to long term care units and works with Social Worker for processing to Finance Department.
8. Signs-in all long term care applicants on admission and copies and distributes paperwork accordingly
9. Performs other duties as assigned.

Minimum Qualifications:

Education: Bachelor’s degree preferred

Experience: 3 – 5 years admissions experience with knowledge of Medicare, Medicaid, and insurance verification
