Responsibilities

  • Coordinate and process admissions inquiries, referrals, applications, and requests for information
  • Gather and review pre-admission information to determine level of care needs and admission eligibility
  • Prepare and complete admission paperwork with residents and families and distribute documentation to appropriate departments
  • Serve as a liaison between hospitals, discharge planners, referral sources, residents, and families
  • Welcome and orient new admissions to the facility and provide tours to prospective residents and families
  • Support facility census goals through marketing and relationship-building efforts
  • Maintain admission records, logs, and documentation in accordance with facility policies and regulations
  • Maintain HIPAA, infection control, safety, and corporate compliance standards
  • Perform additional duties as assigned by supervisor

Admissions Coordinator
Rate: $24 - $26/hour (depending on experience)
Shift: Full-Time

REQUIREMENTS
  • High School Diploma or equivalent required
  • Minimum of 1 year experience working with residents and families in a long-term care setting required
  • Strong communication, customer service, and organizational skills
  • Ability to multitask and work collaboratively with interdisciplinary teams
  • Compassionate and professional approach when working with residents and families
  • Strong problem-solving and follow-up abilities
  • Ability to maintain confidentiality and professionalism in a fast-paced environment
