Job summary

Lightways Hospice and Serious Illness Care in Illinois is seeking an Admission RN to guide patients and families through the admission process. This full-time role involves a 3-shift schedule of 12 hours with rotating weekends, conducting assessments, planning care, and educating families.

The RN will assess hospice needs, develop plans of care, and provide patient and family counseling and instructions per the plan. A valid Illinois RN license and reliable vehicle are required.

Qualifications

• Active RN license in Illinois.
• Minimum two yearsa0years nursing experience; hospice/home health preferred.
• Must have a reliable automobile with adequate insurance.

Responsibilities

• Complete initial and ongoing assessment of the patient and family to determine hospice needs.
• Provide a complete physical assessment and history of current and previous illnesses.
• Identify patient and family needs on admission and initiate the Plan of Care.
• Develop a care plan with goals based on nursing diagnosis and palliative actions.
• Counsel the patient and family in meeting nursing and related needs.
• Provide health care instructions to the patient per assessment and plan.

Job description

Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois. We provide serious illness care, hospice care and grief support for adults and children.

The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices.

Position

Admission RN — We are currently seeking a full-time Admission RN to assist our patients and families through the admission process. The Admission RN evaluates patient for hospice, educates patient and family, orders medications and DME, and starts the plan of care. This position is (3) 12-hour shifts with rotating weekends.

Responsibilities

• Complete an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
• Provide a complete physical assessment and history of current and previous illness(es).
• Identify patient and family needs on admission and initiates Plan of Care activities. Proceeds with dissemination of information to On-Call Nurse or Case Manager.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions (Includes the patient and the family in the planning process).
• Counsel the patient and family in meeting nursing and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan.

Education/Experience

• Registered nurse with current licensure to practice professional nursing in the state.
• Minimum of two years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
