Job summary

Baptist Home Health Care by BAYADA is seeking a compassionate, dedicated RN for Start of Care/Oasis Admissions in Jacksonville, FL. This full-time role conducts home visits across Duval and surrounding counties on a per-visit basis, following established care plans and ensuring safe, high-quality care for adult and geriatric clients.

Requirements include FL RN license and at least 1 year of clinical experience; home health experience is strongly preferred.

Qualifications

• Must hold a current FL RN license.
• Minimum of 1 year clinical (medical/surgical) experience.
• Home health experience preferred but not required.

Responsibilities

• Follow designated care plans for patients in home settings.
• Make home visits to clients in Jacksonville and surrounding territories.
• Administer medications, wound care, treatments, and procedures as prescribed.
• Monitor client conditions and report changes to supervisor.
• Document orders and assessments accurately; coordinate with care teams.
• Maintain timely documentation of observations and interventions.

Registered Nurse (RN) Responsibilities:

• Follow a designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the‑art touch pad tablet

Qualifications for a Registered Nurse (RN):

• A current license as a Registered Nurse in FL.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.
