Job summary

Human Touch Home Health is seeking an experienced Admission Nurse (RN) to provide Start of Care and admission visits in homes across Prince George’s and Montgomery Counties, MD. The role requires independent fieldwork and strong skills in assessments, OASIS documentation, and care planning.

The ideal candidate holds a Maryland RN license, with 1+ year of nursing experience and home health exposure, including SOC and OASIS workflows. Travel throughout the service area is expected.

Qualifications

• MD RN license active and unrestricted in Maryland.
• Minimum one year of nursing experience; home health preferred.
• OASIS and Start of Care experience strongly preferred.
• Strong clinical assessment and documentation skills.
• Excellent communication, organization, and time management.
• Ability to travel across Prince George’s and Montgomery Counties.
• Independent field-based practice.

Responsibilities

• Complete SOC and admission visits in patients’ homes.
• Perform comprehensive nursing assessments.
• Complete OASIS documentation accurately and timely.
• Conduct medication reconciliation and identify concerns.
• Assess patients’ clinical, safety, environmental, and support needs.
• Develop and initiate individualized plans of care.
• Educate patients and caregivers about diagnoses, medications, treatments, and the home health plan of care.
• Coordinate care with physicians and interdisciplinary team.
• Communicate changes in patient condition or concerns promptly.
• Complete required documentation accurately and timely.
• Maintain compliance with Medicare Conditions of Participation, agency policies, and applicable regulations.

Admission Nurse (RN) – Home Health

Prince George’s County & Montgomery County, Maryland

Human Touch Home Health is seeking an experienced and dependable Admission Nurse (RN) to provide home health admission and Start of Care visits throughout Prince George’s County and Montgomery County, Maryland.

We are looking for an RN who is confident working independently in the field and understands that a thorough admission sets the foundation for quality patient care.

Key Responsibilities

• Complete Start of Care (SOC) and admission visits in patients’ homes
• Perform comprehensive nursing assessments
• Complete OASIS documentation accurately and within required timeframes
• Conduct medication reconciliation and identify potential concerns
• Assess patients’ clinical, safety, environmental, and support needs
• Develop and initiate individualized plans of care
• Educate patients and caregivers regarding diagnoses, medications, treatments, and the home health plan of care
• Coordinate care with physicians and members of the interdisciplinary team
• Communicate changes in patient condition or concerns promptly
• Complete required documentation accurately and timely
• Maintain compliance with Medicare Conditions of Participation, agency policies, and applicable regulations

Qualifications

• Current and unrestricted Maryland Registered Nurse (RN) license
• Minimum of one year of professional nursing experience preferred
• Home health experience strongly preferred
• OASIS and Start of Care experience strongly preferred
• Strong clinical assessment and documentation skills
• Excellent communication, organization, and time-management skills
• Ability to work independently in a field-based position
• Reliable transportation and valid driver’s license
• Ability to travel throughout Prince George’s County and Montgomery County
